Providing families with access to quality healthcare

Chorus Community Health Plans (CCHP), an affiliate of Children's Wisconsin, is a health insurance organization dedicated to providing access to the highest quality healthcare and services to our members.

Chorus Community Health Plans offers two products to our Wisconsin members:

• BadgerCare Plus - CCHP is offered in 28 counties in eastern Wisconsin, and covers more than 150,000 adults and children, making it the largest BadgerCare health plan in the state. With the combined resources of Children's Wisconsin, Children's Community Services, Children's specialty and primary care clinics, and our community partners, we are committed to improving the medical and social well-being of our members and the community.
• Individual and Family Plans - In 2016, Chorus Community Health Plans introduced their individual and family plans, on and off-Marketplace. Each plan offers members access to high-quality healthcare from our network of providers across 15 counties in eastern Wisconsin. Our health network includes major hospitals and health systems like Ascension, Bellin Health, Children’s Wisconsin, Door County Medical, Froedtert, Holy Family Memorial, the Medical College of Wisconsin, ProHealth, Rogers Memorial and independent providers as well.
