All families with kids 12 and older can now schedule COVID-19 vaccine appointments online. Open slots can be viewed and selected below. All vaccine appointments are currently held on the Milwaukee campus. Please know that a parent or guardian must be present at the time of vaccination.
If you need help scheduling your appointment, please call (414) 266-3992 (Hours available: 7:30 a.m. – 5:30 p.m., Monday-Friday).
Your appointment for the second dose of the vaccine will be scheduled in the office when you receive your first dose.
If your child received their first dose of COVID-19 vaccine, you must return to that location for the second dose.
Your child needs a well-baby visit or an annual, sports, or camp physical.
Note: Flu vaccine scheduling is now open.
