The purpose of the Children’s Wisconsin Teen Advisory Council is to provide the youth perspective to improve patient experience and quality of care, especially as it relates to the unique needs of teens and young adults. This may occur through surveys, online meetings, in person events, and other opportunities to advise and make recommendations. Members will also be able to develop storytelling and leadership skills and gain experience in listening to and understanding diverse viewpoints.
Who Can Become a Teen Advisory Council (TAC) Member?
- Age 12-21 years old
- A patient of Children’s Wisconsin or a sibling of a patient
- Have experienced multiple interactions with Children’s Wisconsin at any location
- Referral form completed by a provider, staff member, community member, or current council member
- Parent or guardian approval of participation (under 18)
How Do TAC Members Get Started?
Complete this interest form. A TAC staff coordinator will reach out to set up a virtual interview. The person referring the member will need to complete this referral form if they haven’t already.
What Happens After the Interview?
Based on the member’s preference, they will become a virtual or an in-person volunteer.
Virtual volunteering
- Complete the Volunteer Services online application
- Submit agreement to the confidentiality statement.
- Complete Volunteer Services orientation
- Complete the Background Information Disclosure form (18 and older only)
In person volunteering
- Submit the Parental and Employee Health and Wellness consent forms (Under 18 years only)
- Complete the Volunteer Services online application
- Submit official immunization records and agreement to the confidentiality statement.

Note: Members will need to complete annual online training courses to remain a TAC member. Those volunteering in person will also need to receive an annual flu shot.

Is there an application deadline?
No. Applications are accepted at any time throughout the year.
How much do I need to participate?
You will need to attend and participate in at least 50% of regular meetings, surveys, and events. Some opportunities may be limited to certain ages, virtual vs. In person, and other parameters. You must commit to participating in TAC for at least one year.
Can I get credit for my volunteer hours?
Yes! The staff coordinators are happy to complete forms or write a letter attesting to your participation in TAC.
Can I still be a member if I go away to college?
Yes! You can be a member as long as you meet the requirements of the TAC and maintain an active volunteer status.
How do I access the online application after my interview with the staff coordinators?
The Volunteer Services Coordinator will provide you with the online application link once consent forms are submitted.
