Iron deficiency remains the most common cause of anemia in children, but not every case needs a hematology consult — most respond well to a straightforward oral iron trial in primary care. However, now safe and effective IV iron options are available, and insurance typically will approve when necessary. The American Society of Hematology (ASH) and the American Society of Pediatric Hematology/Oncology (ASPHO) recently updated target ferritin thresholds to reflect better understanding of iron metabolism. To refer a patient:
