Make a referral
The Center for Child Development does not require a physician referral to see a patient, unless a referral is required by insurance.
Families may call the Mental and Behavioral Health intake line 414-266-3339, option 2, to request an appointment in the Center for Child Development. Physicians may also enter a referral order in Epic or fax referral information to us at (262) 432-6604.
Families who are referred to us should call the Mental and Behavioral Health Intake line to provide information regarding their child’s needs. We also request relevant background information such as Birth-to-Three records and/or an IEP, to schedule the patient with the most appropriate provider(s). Please sign and print the medical records release form and email it to the Center for Child Development at ChildDevelopment@childrenswi.org so that we may request the IEP from your child's school.

Please Note: The Center for Child Development is primarily an evaluation clinic. If a patient is having behavioral issues that cannot wait for an evaluation, please contact a local behavior therapy provider. Community resources often have openings within the week. If you need a list of therapy referral sources, please contact us.
