Adriana de Moya, MD, pediatrician, Children’s Wisconsin Primary Care.

About the Forest Home Clinic

The Forest Home Clinic will open on Monday, June 20. Children’s Wisconsin is proud to offer more accessible and convenient care for families on the south side of Milwaukee. The clinic adds to the physical presence of Children’s Wisconsin  in the neighborhood, which included operating an urgent care clinic for more than 20 years at Sixteenth Street Community Health Center. In addition, Children’s Wisconsin supports the near south side with school nurses and two community navigators, as well as insuring many residents through Children’s Community Health Plan (CCHP).

Located at the intersection of Forest Home Avenue and 15th Street, the Children’s Wisconsin Forest Home Clinic will provide comprehensive pediatric care including primary care, urgent care, dental, imaging, family and child therapy (behavioral health), speech therapy, neurology, pulmonary medicine and post-surgery appointments.
