Endocrinology referral tips

Urgent referrals
For patients you identify as needing to be seen within 1-2 weeks, contact the outpatient endocrinologist on call via the Children's Wisconsin Physician Referral and Consultation line: (800) 266-0366.

New diabetic patients
Contact the outpatient endocrinologist on call via the Children's Wisconsin Physician Referral and Consultation line at (800) 266-0366. Download the ADA Guidelines for information on the diagnosis of diabetes (PDF).

Routine referrals
Have the family schedule an appointment by calling Central Scheduling at (877) 607-5280. The family member will need to know the reason for the referral so the patient can be scheduled in the appropriate clinic. Download a list of diagnoses/problems typically seen in our clinics (PDF).

Overweight, rapid weight gain, or obesity
While we do not offer a weight-loss program, we are happy to assist with the medical evaluation for obesity. Note that these visits may not be covered by the patient's insurance. Download information about these referrals (PDF).

Hyperlipidemia
Download information and guidelines about hyperlipidemia referrals (PDF).

Please fax all pertinent records, including growth charts and results of any diagnostic testing to (414) 266-6749.

We are happy to discuss your patients with you. If you would like to speak with a provider prior to making a referral, call the outpatient endocrinologist on call via the Children's Wisconsin Physician Referral and Consultation line: (800) 266-0366.
