Camp Oak Leaf
Camp Oak Leaf is for Children’s Wisconsin families who have experienced the death of a child. All family members, infant through adult, are invited to attend. Camp Oak Leaf provides a balance of fun activities (sports, games, art, etc.) with grief education & projects that are age appropriate for each member of the family. Families will have the opportunity to have the camp experience, while having access to modern comforts. Families sleep in a dorm like setting, each room has its own private bath. Our hope is the weekend's activities will help family members heal and connect with each other, find comfort in connecting with other grieving families, and provide shared experiences that will support them in their grief journey in the days, months and years to come.

Jewish Community Center Rainbow Camp

Buses will transport families from the Children’s Corporate Center - 999 N. 92nd St, Wauwatosa, 53226. Buses will leave Children’s at 8:30 a.m. on Saturday. Buses will arrive back at Children’s at approximately 2:00 p.m. on Sunday.
Families may choose to drive to Camp Oak Leaf on their own.

2023 Camp Oak Leaf - Albert and Ann Deshur JCC Rainbow Day Camp
I understand that this is a camping program of the Harry and Rose Samson Family JCC and Children’s Wisconsin. I grant permission for myself and child(ren) to participate in this camp and its activities, for the camp to obtain or provide medical treatment for myself and child(ren) in an emergency and for the camp to provide myself and child(ren) with transportation.
I also grant permission for the use and publication of myself and child(ren)’s picture, video-recording and likeness in advertising, promoting and publicizing Rainbow Camp. I understand that no monetary benefits will be provided to me or my child for use of myself or child(ren)’s picture, video-recording or likeness.
Release of Liability
I understand that my participation and my child(ren)’s participation in this camp may involve some risk including injury or illness, with losses which may result not only from myself or child’s own actions, inactions or negligence, but also from the actions, inactions, or negligence of others. I certify that myself and child(ren), who may have a chronic health condition is in good health, and there is no medical reason preventing myself or child(ren)’s safe participation in this activity. In consideration for myself and child(ren)’s participation in the activity, I agree to forever waive all claims and causes of action against Children’s Wisconsin, Inc., the Harry and Rose Samson Family JCC, their affiliates, directors, officers, employees, volunteers and agents and release from all liabilities, demands, claims, losses, costs or damages arising out of or related to my participation or child(ren)’s participation in Rainbow Camp.
I have read this waiver and release agreement and understand the terms used in it and their significance. This waiver and release is freely and voluntarily given with the understanding that right to legal recourse is given up in return for allowing myself and child(ren)’s participation in Rainbow Camp. My signature on this document is intended to bind myself and my successors, heirs, representatives, and assigns.
