New Laboratory Order Form
Children’s Wisconsin has improved our outpatient laboratory order form. This is now a fillable form that can be complete before printing, signing and faxing. The updated form includes changes in allergy testing, and continues to include commonly ordered tests of blood, urine, stool, cultures and drug levels.

Please discontinue using all previous versions and share this news with any office staff that helps send lab orders to Children’s.
