General Consent Form

The Children’s Wisconsin General Consent form (routine hospital or outpatient treatment) covers services within Primary Care, Ambulatory/Specialty Care, and Ancillary/Diagnostic services for a rolling 12 months. Download the outpatient consent for treatment (primary care, specialty care and ancillary services) forms:

Finding your clinic

Welcome Centers are located throughout the hospital and in Children’s Clinics Building. Welcome Center staff can help you with directions.

Identification bands

An identification band will be placed on your child as soon as he or she is admitted to the hospital. Our staff must use that band to check your child’s identity before all procedures, including giving medication or giving blood. This is an important safety check to make sure your child gets the correct treatment.

Visitor badges

Identification badges must be worn at all times by family members and visitors in the hospital. A parent/grandparent/guardian badge will be given only to adults who are directly responsible for the patient. A visitor badge will be given to others on the visitor list. Badges are available at our Welcome Centers and nurses’ stations. A new badge is needed every day. They can be turned in to a Welcome Center when you leave.

Visitor list

Parents or guardians create a visitor list when their child is admitted. Only the parent or guardian can add names to the list. All visitors, including family members, must be on the list to visit your child. For safety reasons we may remove people from the list. These people will not be allowed to visit. Some units have restrictions on visitors. Check with your child’s nurse.

Visiting hours

Visiting hours are from 8 a.m. to 8 p.m. Parents, grandparents and guardians may visit anytime.
