Information About Emotional Health Screening at Children’s Wisconsin

At Children’s Wisconsin, we believe caring for a child’s mental and behavioral health is just as important as caring for their physical health. We screen children ages 5 and older with a PROMIS questionnaire at Primary Care visits.

PROMIS stands for Patient-Reported Outcomes Measurement Information System. This tool helps us understand your child’s emotional health and well-being at each visit. It allows us to check and track a child’s physical, mental and social health by focusing on their experiences. These checks are done through simple questions that ask about the child’s experience over the past seven days.

Children over age 10 should answer the questionnaire themselves so their responses reflect their personal experiences, feelings and well-being. Answers are confidential and used solely to improve your child’s care. Results are reviewed together with you and your child. This screening helps us spot concerns early, track changes over time, improve communication about your child’s well-being and provide more personalized care.
